Prior to any athletic participation in a UIL/LCISD athletic activity – required forms must be filled out and returned to the Campus Athletic Trainer.

We encourage all of our athlete's to travel to and from athletic events with the team. If a need arises and you will need to pick your child up from an athletic event, you will need the Parent Travel Permission Form filled out prior to the event.
