IMPORTANT HEALTH NOTICE FOR PARENTS OF
NAVARRO MIDDLE SCHOOL 6TH GRADE STUDENTS
Attention Navarro Middle School Parents:
This letter is to inform you that there are ADDITIONAL vaccine requirements for students who will be entering 7th grade next year. The ADDITIONAL requirements are:
To ensure a smooth transition from 6th to 7th grade, please make sure that your student’s immunizations are updated with the new requirements, AND on file in the Navarro Nurse’s Clinic AS SOON AS POSSIBLE. A doctor or clinic signature and/or stamp is required to validate each vaccine received. Once received by the Navarro MS Nurse, your student’s health record will be updated, then forwarded to the junior high school that he/she will be attending next school year.
Please be aware THAT FAILURE to provide the 6th Grade Nurse WITH UPDATED vaccine records this year will result in your child being unable to attend classes in 7th grade next year until the immunization records are complete!!!!!!!
Immunizations can be obtained from: your private physician, the Fort Bend County Health Department (281-342-6414), Access Health (281-342-5176), or, for eligible students, the Memorial Herman School Based Health Clinic at Terry High School (281-238-0859).
